Account Forms
The following forms can be used for accounts with the Minnesota United Methodist Foundation. Completing the forms online may be dependent upon the browser you are using and you may have to first download the form and then complete it and forward it on to us. Address information is provided at the very bottom of the webpage.
OPEN A NEW FOUNDATION ACCOUNT
To open a new account at the Minnesota United Methodist Foundation, your organization should complete and submit the following two forms to the Foundation:
DEPOSIT FUNDS TO YOUR FOUNDATION ACCOUNT
Mail your check for deposit to your account directly to the pooled accounts with the Wisconsin United Methodist Foundation. If your organization has multiple accounts at the Foundation, you can include a completed deposit slip with your check, to insure that your deposit is directed to a specific account:
REQUEST WITHDRAWALS FROM YOUR FOUNDATION ACCOUNT
To request funds from your Foundation account, please complete a withdrawal request form. Obtain an appropriate signature on the form, and either mail, e-mail, or FAX the form to the Minnesota United Methodist Foundation. Withdrawals are only processed at month-end (12 times per year). We must receive your completed withdrawal request form in our office by the 20th of the month in order to process it for the upcoming month-end. RECEIVE DIRECT DEPOSITS INSTEAD OF CHECKS
To have all future withdrawals from your Foundation account electronically deposited directly to your organization’s bank account, please complete and submit the following form to the Foundation:
VIEW YOUR ACCOUNT ON-LINE
To have monthly on-line access to the activity of your account instead of receiving printed quarterly statements, please complete and submit the following form to the Foundation:
